Physical Examination

 

Vitals:

BP:  138/88 sitting, 120/70 standing; HR:  64, irregularly irregular; RR:  16; Temp:  37.0o

 

CVS:  Normal S1 and S2, no S3 or S4, no murmurs.  JVP 3cm above the sternal angle.  See below for comments regarding lower extremities.

 

Resp:  Chest clear to bases.  Normal breath sounds.  No adventitious sounds.  No accessory muscle use.

 

Legs

 

Right leg – inner aspect of calf, gaiter area, ulcer of approximately 3cm by 4cm, shallow, granulation tissue present.  Mild serosanguinous exudate.  Mild surrounding redness, no heat detected.    Chronic venous changes including hemosiderin deposits noted.  Dorsalis pedis and posterior tibialis pulses not detected.  Capillary refill 2 seconds.  Edema 2+ to knee, pitting.

 

Left leg – similar to right leg, except smaller ulcer (3cm by 3cm). 

 

The ulcers have dry dressings on them.
