Isolated medial malleolus fracture
The radiographs below demonstrate an isolated undisplaced fracture of the medial malleolus. This fracture likely occurred during forced eversion or external rotation of the foot. The resulting tension on the deltoid ligament avulsed the tip of the medial malleolus. These fractures can occur in isolation but are often associated with lateral malleolus fractures, posterior malleolus fractures or proximal fibular fractures (Maisonneuve's fracture). The radiographs should be carefully reviewed fro these other fractures and the patient carefully examined for proximal fibular tenderness. This particular fracture can be managed with a below knee cast for 6-8 weeks. If associated with other fractures, emergent orthopedic consultation is indicated.
